Rise Up! ExercisAbilities Balance Enhancement Program
10 week Balance and Stability Program
Sessions 2x/ week for 8 weeks and 1x/ week for 2 weeks.
This program has been designed to utilize traditional research-proven methods for fall risk reduction and programming as well as incorporate more advanced higher-level balance testing and training methods available at Exercisabilities and individualized to each client to meet their unique needs.
Instruction is 1:1 with the therapist and client. Insurance or private pay is utilized for program participation.
Some key components of this program:
Professional strength, balance, mobility pre, mid, and post-program assessment by a trained physical therapist
Introductory information: “What to expect from this program, Exercise is important for older Adults, Benefits, and Barriers to Exercise, What is your level of Exercise, Choose the Right Exercise Plan for You, Creating Healthy Habits, Are you at Risk Questionnaire, My Falls Free Plan, Information for Primary Caregivers”
“My Program Goals” Goal setting individualized to meet client and family/caregiver priorities as well as those advised by the therapist
Guided Safe and effective strength and general fitness training set to your level of ability and comfort
Higher-level balance pre/ post-program assessment and training utilizing the computerized Humac Balance System
The therapist guided the individualized selection of optimal research-proven strength and balance exercises
Fall risk multifactorial assessment and education addressing individual needs such as: “Vision and Fall Prevention, Medication Management, Choosing Optimal Footwear, Assistive Devices Can Help, Safety Alarm Systems, Make Your Home Work for You and Home Safety Checklist, Getting Out and About, How to get up after a Fall”
Class packet and weekly materials, handouts provided as part of the program
Discharge recommendations and guidance for additional local and community-based resources or beneficial classes
